1. My office is located about 1/3 mile West of the 101 freeway, on the southeast corner of River Street and Broadway Road (between Price and McClintock).
The address is:
2101 E. Broadway Road, Suite #25
(on the south side of the complex)
From the loop 101 freeway, take Broadway west to River Street. Turn left (south) on River and immediately turn left into the complex driveway and park anywhere. Suite #25 is just a few feet away on the south side of the building..
Most major insurance plans other than Cigna are accepted for payment .
Please call to confirm or consult your insurance company for details. My standard fee is $90/session $100 for the initial visit.
